Required Photo Notary Needed with Online License Application Submission
The ND-100_P_N Application Photo Notary form must be completed if you are filing an application online. The photo must be a 2”x2” passport quality photo. The photo must be taken within 6 months of the application filing date and should be of your head and shoulder area only.
